H2 Performance focuses on athletes and active individuals through sports rehabilitation, performance training, and softball development. The emphasis is on injury recovery, movement optimization, performance progression, and long-term athletic durability through a collaborative 1:1 model.

H2 Rehab provides in-home physical therapy services for individuals requiring longer-term or more functional care, including post-operative recovery and adult and pediatric neurological rehabilitation. This model prioritizes accessibility, independence, mobility, and improving quality of life within the home environment.

Meet The Team

  • Kacyee Hart, PT, DPT, is an experienced physical therapist with a focus on sports rehabilitation and neurological disorders. As a former Division 1 softball player, she combines firsthand athletic experience with professional expertise to provide personalized, evidence-based care.

    Kacyee earned her Doctor of Physical Therapy degree from Washington University in St. Louis in 2019. She has gained valuable experience at the renowned Shirley Ryan AbilityLab, where she worked with diverse adult and pediatric neurological populations. Additionally, Kacyee has trained at sports rehabilitation facilities affiliated with Creighton University and Champion Sports Medicine in Birmingham, Alabama, where she gained specialized expertise in athlete rehabilitation and recovery ranging from pediatric to professional skill levels. Her broad experience in these settings allows her to effectively address the unique needs of athletes and individuals recovering from injury. Kacyee’s expertise in neurological rehabilitation with her background in sports therapy provides well-rounded, holistic care that addresses both the physical and functional needs of her patients.

    Kacyee is dedicated to helping her patients achieve their physical goals by empowering them with effective treatment strategies. Beyond her clinical work, she is passionate about training young women in weight lifting techniques. She believes that building strength not only improves athletic performance but also fosters self-confidence and resilience. Kacyee emphasizes the importance of proper lifting form to prevent injury and promote lifelong health habits. Through her training programs, she strives to inspire young women to adopt sustainable fitness practices that support their overall well-being, both in and outside the gym

  • With over 40 years of experience, Lisa Hart, PT, is a highly skilled physical therapist specializing in both neurological and orthopedic rehabilitation. She earned her degree from Northwestern University, where she built a strong foundation in advanced therapeutic techniques. Throughout her career, Lisa has remained dedicated to improving the quality of life for her patients through individualized, evidence-based care. Her extensive expertise and hands-on experience allow her to effectively treat complex conditions, helping patients regain functionality and achieve their rehabilitation goals.

    In addition to her successful private practice, Lisa trained at the prestigious Rehab Institute of Chicago, now known as Shirley Ryan AbilityLab, renowned for its pioneering research and advancements in rehabilitation. There, she embraced a multidisciplinary approach that blends innovative treatment strategies with a focus on patient-centered care. This experience further honed her skills in addressing diverse patient needs and deepened her commitment to evidence-based practices.

    Lisa has also gained significant experience in pediatric rehabilitation, working with children across a range of neurological and orthopedic conditions. Her compassionate, tailored approach helps children improve motor skills, mobility, and overall function, while also supporting families through the rehabilitation journey. Lisa’s experience in pediatric therapy allows her to provide age-appropriate care that focuses on both physical development and emotional well-being, ensuring the best possible outcomes for young patients.

    Lisa also served as a rehabilitation specialist for the Chicago White Sox, bringing her expertise to professional athletes and developing tailored rehabilitation programs to meet their unique physical demands. Her background in sports rehabilitation enhances her ability to help patients—athletic or otherwise—reach their full potential through effective, personalized therapies.

She is thorough, explains everything that she is doing and why, and incorporates both therapy and the sport into the sessions. My daughter was suffering from a lower back injury that would not go away. It was impossible for her to pitch, even minimally, without pain. As a former D1 pitcher, Kacyee knows the ins and outs of pitching mechanics and could address her pain issues not only through PT exercises, but also through adjusting her mechanics while pitching. We will continue to work with Kacyee outside of PT so that we can continue to monitor my daughter's physical health and keep her pitching efficiently and (hopefully) pain free!   • "Kaycee has been an important part of Reagan’s recovery the past year. She had PT over last winter to address a foot/toe issue that only seemed to strengthen everything around the foot but not attack the root cause. Reagan had been working through the pain for 9 months before we found another path. When meeting Kaycee, she took measurements on flexibility in the foot and toes and fully understood what Reagan needed to address as a softball pitcher since Kaycee was a D1 pitcher herself. A couple weeks ago, Reagan pitched her first complete 7 inning game in 11 months and struck out 7 batters for the win. As an added bonus, she has her power back in her swing and is hitting the ball much harder again! We will do more sessions with Kaycee once fall ball is over. I highly recommend using Kaycee for your sports injury needs!"

  • We are currently out of network with some insurance providers in order to focus on delivering quality care with one-on-one treatment sessions. By not being tied to the constraints of certain insurance networks, we can offer a more personalized, tailored approach to your care, ensuring that your needs are met with the attention they deserve.

    While being out of network may impact your insurance coverage, we do provide a superbill upon request, which you can submit to your insurance provider for possible partial reimbursement. Our team is also available to help you navigate the process and explore alternative options.

  • We understand that out-of-network care can be a concern, and we want to make the process as easy as possible for you. Here are some ways you can cover the cost of services:

    1. Submit a Superbill for Reimbursement
      After each session, we provide a superbill which includes all the necessary details to submit to your insurance provider for possible reimbursement. Many insurance companies offer out-of-network benefits, so it’s worth checking with them to see if they will cover part of the cost.

    2. Health Savings Accounts (HSA) & Flexible Spending Accounts (FSA)
      If you have an HSA or FSA, you can use those funds to pay for your treatments. These accounts are often used for medical expenses and can be a convenient way to cover services.

    3. Discounted Packages
      We offer discounted treatment packages for clients who commit to multiple sessions. These packages provide a lower per-session rate and can help make ongoing care more affordable.

    4. Ask About Sliding Scale Fees
      If you're facing financial hardship, please reach out. We are committed to providing access to care and may offer sliding scale fees or other options depending on your situation.
